TO: JOHN DOE, unknown biological father or any person claiming to be the father of T.S.

PLEASE TAKE NOTICE that on January 17, 2023, at 9:00 o’clock a.m, the petitioner, the West Virginia Department of Health and Human Resources, is seeking to determine that JOHN DOE, unknown biological father of T.S., has caused such child to be in imminent danger and further to adjudicate that they are abusing parents, and to further seek to terminate the parental and custodial rights of JOHN DOE. The petitioner alleges, among other things, that health and welfare of T.S. is harmed or threatened by a present refusal, failure or inability of JOHN DOE, to supply the children with necessary food, clothing, shelter, supervision, medical care or education.
It is ORDERED that an adjudication/disposition hearing be held before the Honorable Jason A. Cuomo, Judge of the Circuit Court of Brooke County, West Virginia, Third Floor, Brooke County Courthouse, 632 Main Street, Wellsburg, West Virginia on January 17, 2023, at 9:00 o’clock a.m, and that JOHN DOE., appear to object or make other defenses to the disposition requested herein. Failure to appear or otherwise make defense at such hearing may result in the termination of the parental and/or custodial rights of JOHN DOE, or any person claiming to be the biological father of T.S.
